Job summary

Valley Children’s Healthcare is seeking a part-time nurse navigator/case manager to assess, plan, implement, coordinate, monitor, and evaluate patient health needs across the continuum of care within the SNF. The role requires a CA RN license, pediatric experience, and CM/UM exposure, with bilingual skills preferred.

Responsibilities include collaborating with the healthcare team to ensure appropriate utilization of resources and timely care delivery, while maintaining compliance with nursing

Qualifications

• RN - Registered Nurse License - CA-BRN - California Board of Registered Nursing (required)
• Minimum two (2) years Nursing experience (required)
• Pediatric experience and Case Management/Utilization Management experience desirable.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate options and services for patients.
• Coordinate services for a defined patient group ensuring appropriate setting and intensity of care.
• Collaborate with the care team to meet outcomes and resource utilization within expected time-frames across the continuum of care.
