Job summary

By The Bay Health in San Francisco, CA seeks a Pediatric Nurse Case Manager for a per diem role. The nurse will assess and plan care for pediatric patients and coordinate with physicians and the hospice team to maximize comfort and health for children and families.

The position requires a CA RN license and at least one year of pediatric nursing experience; ELNEC training is preferred and must be obtained within the first year.

Qualifications

• One year of professional nursing experience within the past three years.
• One year of pediatric nursing experience.
• Experience in acute care/home health and/or hospice setting preferred.

Responsibilities

• Assume primary responsibility for pediatric patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions.
• Apply admission and re-certification criteria for hospice care to pediatric patients.
• Initiate communication with attending physicians, hospice staff, and other agencies to coordinate care.
• Maintain regular communication with the attending physician regarding patient/family care needs and response to interventions.

As part of the pediatric team, the Pediatric Nurse Case Manager will provide skilled nursing assessment, planning, and care to maximize the comfort and health of pediatric patients and families in compliance with organization policies and procedures and applicable laws and regulations.

Schedule

1 day per week (days are flexible); 8:30am - 5:00pm

Location

San Francisco with additional coverage to Contra Costa Counties as needed.

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

• Assume primary responsibility for pediatric patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• As applicable, apply specific criteria for admission and re-certification to hospice care or hospice waiver care to establish appropriate levels of care and patient’s eligibility for children
• Initiate communication with attending physicians, other hospice staff members, facility staff members as applicable, and other agencies as needed to coordinate optimal care and use of resources for the patient/family
• Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions

Education/Experience

• One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
• One year of pediatric nursing experience
• Experience in acute care/ home health and/or hospice setting preferred

Certificates, Licenses, Registrations

• Current license as a registered professional nurse in the State of California
• Public Health Nursing certification highly desirable
• HPNA certification preferred
• ELNEC Pediatric training preferred and required within the first year of employment

Pay Range

$58.90 - $73.65 hourly
