Job summary

Every Child Pediatrics in Denver seeks a full-time Care Navigator to coordinate patient care and resources at our clinic, Monday–Friday, in person at 1601 E 19th Ave, Suite 6600, Denver, CO 80218.

The role focuses on guiding families through medical and community systems, empowering autonomy, and collaborating with clinicians to ensure timely access to services and support. We value bilingual communication and compassionate care.

Qualifications

• Bachelor’s degree required; associate degree with related experience may be considered

Responsibilities

• Help families access and coordinate medical specialty care, community services, developmental assessments, and school planning
• Build trusting relationships with patients and families through phone calls, texts, and in-person meetings at the clinic, community spaces, schools, or homes
• Serve as the primary point of contact, advocate, and informational resource for patients/families and community partners/payers
• Work with healthcare providers in a team-based approach to identify patients with special and complex needs and initiate the care plan process
• Develop, organize, and maintain up-to-date, comprehensive, culturally responsive resources for patients and families
• Maintain accurate, thorough, and timely records of all activities and provide data for reports
• Treat all clients with dignity and respect and honor their right to privacy

Education
