Job summary

AdventHealth in Orlando, Florida, is seeking a Medical Social Work professional to support discharge planning. You will assess patients and families holistically, review records, and coordinate post-acute care to ensure timely transitions.

The role requires a Master’s in Social Work, strong communication, and ability to collaborate with multidisciplinary teams. You will participate in rounds, educate families, and advocate for patient rights.

Qualifications

• Master’s degree in Social Work required.
• 2+ years of social work experience required.
• 2+ years of care management experience preferred.
• ACM/CCM certifications preferred.

Responsibilities

• Assess patients and families holistically for discharge planning needs.
• Review medical records and incorporate clinical, social, and financial factors into the discharge plan.
• Develop discharge plans and coordinate post-acute care services and facilities.
• Communicate with post-acute care facilities to ensure continuity of care and accurate medical records.
• Participate in multi-disciplinary rounds to review patient status, discharge plans, and required resources.
• Educate patients and families about illness, psychosocial impacts, and available resources.
• Organize and facilitate patient-family care conferences with the multidisciplinary team.
• Document evaluations, discharge plans, barriers, and progression of care as required.
• Advocate for patient rights and preferences during hospitalization.
• Obtain payor authorization for post-acute care as needed.
