Job summary

Children’s Hospital of Philadelphia (CHOP) seeks a skilled social worker to join our pediatric care team. The role focuses on needs assessments, coordinating community referrals, and supporting patients and families to navigate resources and transitions of care.

You will collaborate with multidisciplinary staff, document activities in the medical record, and assist with insurance pre-authorization and utilization management for eligible cases.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Behavioral Health, Sociology, or similar field required.
• At least three years of experience in a related discipline required.
• Healthcare experience or community resource agency preferred.

Responsibilities

• Perform needs assessments and coordinate community-based referrals.
• Communicate assessments with health care team and families.
• Coordinate patient disposition and referrals in collaboration with team.
• Document activities in the medical record and ensure accurate records.
• Assist with insurance pre-authorization and utilization management for eligible cases.
