Responsibilities

• Conduct needs assessments and provide social work referrals to address identified concerns.
• Communicate assessments and collaborate with health care team, family, and managed care organizations.
• Collaborate with area social workers to support discharge planning and resources.
• Develop psychosocial support plans considering patient, family, and community resources.
• Coordinate understanding of care plan and transition to next level of care or home.
• Identify and coordinate recommended treatment and community referrals.
• Establish relationships with service providers to ensure patient needs are met.
• Engage in process improvement with internal and community partners to improve outcomes.
• Document activities in the medical record.
• Provide insurance pre-authorization and utilization management for behavioral health services.
