Job summary

Boston Children's Hospital seeks a Pediatric Cardiac and Pulmonary Transplant Clinical Pharmacist to join a busy transplant program. You will provide comprehensive pharmacotherapy, optimize immunosuppression, monitor drug interactions, and educate patients and families throughout the transplant continuum.

Collaborative care with physicians, nurses, and the transplant team, with CMS and regulatory readiness as part of ongoing quality improvement and documentation.

Qualifications

• Pharmacy degree plus up to one year post-degree coursework or internship; 2–3 years general pharmacy experience, or 1 year pediatrics.
• Current MA license/registration in Pharmacy.
• Strong analytical and written communication skills.

Responsibilities

• Completion of chart/med review for all heart/lung transplant patients daily.
• Attendance at Cardiac and Pulmonary transplant rounds at least 2 days per week.
• Participation in transplant patient evaluation meetings.
• Document evaluations in medical records; communicate plans to clinical team.
• Documentation of Admission Notes in EMR with transplant plans.
• Pharmacy Consultation and Drug Information as SME for ALL SOT Teams & Transplant Nursing Staff weekdays.
• Interpretation of medication orders to ensure appropriate drug and dose; contact physician as needed.
• Optimization of immunosuppressive regimens, including induction, maintenance, and rejection management.
• Documentation of medication history.
• Participation in quality improvement activities.
• Discharge education and EMR medication teaching.

Education

Bachelor's degree in Pharmacy

Doctorate degree in Pharmacy (Pharm D) preferred

Massachusetts Pharmacist License

Analytical and written communication skills
