Urban Health Plan seeks a Behavioral Health Clinician (Psychologist) to launch our TEAM First program in pediatric behavioral health. The role provides assessment, diagnosis, and brief therapeutic services for children 0–12 and their families within a primary care setting, emphasizing culturally responsive and trauma-informed care.

The clinician collaborates with primary care teams, coordinates referrals, and educates families on behavioral health strategies to improve overall health outcomes.

Qualificações

• Ph.D./Psy.D and NYS Licensure as LCSW.
• Bilingual English/Spanish is required.
• Minimum of 2 years psychotherapy experience with pediatrics (birth to 12) and family support.
• Clear skills in diagnosing pediatric patients.
• Early Intervention/Early Childhood background is a plus.

Responsabilidades

• Manage an assigned caseload of patients, providing behavioral health assessment, brief intervention, and follow-up for children 0–12 in primary care.
• Conduct timely, evidence-based assessments to identify concerns, diagnose, assess risk, and develop treatment recommendations.
• Provide brief, evidence-based therapies to improve functioning and document BH care plans in the EHR.
• Refer to specialty behavioral health when needs exceed integrated care.
• Collaborate with primary care providers, Community Health Workers, and the care team on treatment plans.
• Offer behavioral health expertise to the integrated care team to improve outcomes.
• Educate patients, families, and staff on conditions, options, prevention, and self-management.
• Use validated tools to monitor progress and support population health initiatives.
• Coordinate care with schools, community agencies, and specialty providers for continuity.
• Maintain accurate documentation of assessments, interventions, and progress in the EHR.
• Apply culturally responsive, trauma-informed, and family-centered approaches in planning.
