Job summary

Community Memorial Health System is seeking a full-time Pediatric Behavioral Health Interventionist to integrate behavioral health services into pediatric primary care. The role supports the Collaborative Care for Youth Innovation grant and focuses on screening, brief intervention, and care coordination for ages 0–26.

The position works within a Collaborative Care Model and PCBH framework, partnering with PCPs and behavioral health professionals in outpatient clinics.

Qualifications

• Master's degree required in social work, counseling, psychology, or related field.
• One year of relevant healthcare/behavioral health experience preferred.
• Current driver license and MVR acceptable by CMH insurance coverage.

Responsibilities

• Provide screening, assessment, and brief intervention for behavioral health needs in pediatric primary care settings.
• Coordinate care with PCPs, psychiatrists, and other behavioral health professionals.
• Support families with referrals and transitions across child, adolescent, and young adult populations (0–26 years).
