Qualifications

• RN with BSN required for this role.
• GA RN licensure or multi-state licensure acceptable with GA.
• Current CHOA RN with ADN must obtain BSN within 2.5 years of hire.
• Pediatric behavioral/mental health experience preferred.

Responsibilities

• Coordinate assessment and discharge planning for behavioral/mental health patients.
• Develop and implement transition care plans with the clinical team.
• Communicate with providers, social workers, and payors to drive care coordination.
• Oversee referrals to psychiatric residential treatment facility (PRTF) and coordinate community resources as needed.
• Facilitate care conferences, interdisciplinary rounds, and EMS-related escalations.
