Reporting to the Behavioral Health Support Supervisor, you will lead coordinators, collaborate with pediatric psychiatrists, schools, and community partners, and help connect families to long-term mental health resources through

Qualifications

• Requires a Bachelor's Degree in Social Work, Psychology, Education, or a related field with at least 5 year(s) of experience in a health care or educational setting (preferably K-12).
• Proficiency in electronic systems including Google Suite, Trayt, Slack, Athena or Epic, Calendly.
• Relevant education and experience may be substituted as appropriate.

Responsibilities

• Connects patients and families to mental health resources for long-term care.
• Coordinates with schools, families, and professionals to reduce barriers.
• Maintains confidentiality in all aspects of care.
• Oversees projects and onboarding for new coordinators.
• Leads internal referral pathways and education.
• Collects data and prepares monthly reports for oversight.
