Job summary

UNC Health is expanding social work support in the Pediatric Primary Care Specialty Clinic at Carolina Point II to enhance patient and family access, care coordination, and psychosocial support. The Social Worker – Ambulatory II is an integral member of the healthcare team, conducting assessments, developing patient-centered care plans, and coordinating resources.

Schedule: Monday through Friday, 8:30am - 5:00pm, with extended evening coverage to 7:00pm two days per week.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Licensed Clinical Social Worker (LCSW) in North Carolina.
• Minimum of three years' broad clinical healthcare experience with complex medical conditions.
• Ability to collaborate on an interdisciplinary team and maintain patient confidentiality.

Responsibilities

• Assessments: Conduct biopsychosocial assessments to evaluate patient needs and barriers.
• Patient-Centered Care Plans: Develop individualized plans with the care team.
• Resource Facilitation: Connect patients to community resources and referrals.
• Documentation and Billing: Document and bill for services per requirements.
• Crises Management: Provide crisis intervention and safety planning.
• Progress Monitoring & Evaluation: Track progress and adjust care plans.
• Advocacy: Ensure access to treatment and coordinate communication.
• Active Listening: Provide a safe space and empathy.
• Motivational Interviewing: Use to support health behavior change.
• Respect for Individuality: Tailor care to values and culture.
• Building Trust: Foster therapeutic alliance and confidentiality.
• Team meetings & training: Participate in quality initiatives.
• Consultation: Serve as a resource for team members.
