Job summary

Qualifications

• Provides nursing care using a Family Centered approach.
• Assesses individualized patient needs and plans, implements, educates and evaluates care using Evidence-Based Practices.
• Able to ensure clinic processes and workflows comply with regulatory and policy requirements.
• Works closely with providers and other team members to ensure smooth clinic flow and positive patient/family outcomes.
• Documents according to organizational standards, policies and procedures.

• Collaborates with multidisciplinary team to optimize patient outcomes.
