Qualifications

Master’s Degree in Behavioral Health field (e.g., Professional Counselor; Clinical Social Work; Marriage and Family Counselor; Substance Abuse Counselor)
BLS certification through American Heart Association or American Red Cross
Licensed by AZ Board of Behavioral Health Examiners; DPS Level One Fingerprint Clearance Card; Valid US driver’s license

Responsibilities

Functions as a consultant, clinician, and mentor in delivering and evaluating behavioral health care services.
Perform assessments and develop care plans aligned with patient-centered goals.
Document in EMR and coordinate with patient care teams to ensure timely, quality care.
