Qualifications

• In-depth knowledge of the MDS assessment process, coding guidelines, and PDPM.
• Experience in MDS coordination and familiarity with MDS software systems.
• Strong PDPM understanding and its impact on reimbursement in the SNF setting.
• Excellent analytical skills and attention to detail in assessing, coding, and documenting resident information.
• Proficient in using computer software and applications for data management and reporting.

Responsibilities

• Oversee the MDS process and ensure compliance with regulations.
• Coordinate and manage the completion of the MDS assessments, including initial, significant change, quarterly, and annual assessments.
• Collaborate with the interdisciplinary team to ensure accurate coding and documentation to support PDPM reimbursement.
• Review residents' medical records and observe conditions to gather necessary information for MDS assessments.
• Develop individualized care plans based on MDS assessments and resident needs, and ensure regular reviews.
• Conduct audits and QA checks on MDS assessments and documentation.
• Work with nursing, therapy, social work, and other professionals to gather information and ensure comprehensive assessments.
• Analyze MDS data to identify reimbursement optimization opportunities with the finance team.
• Provide staff education on MDS requirements, coding guidelines, and PDPM best practices.
• Ensure regulatory compliance and implement updates in MDS processes.
• Collect and analyze data related to MDS, reimbursement, and resident outcomes; generate management reports.
