Job summary

The Milton Home is seeking an experienced MDS Coordinator to lead the RAI process and drive accurate PDPM reimbursement. You will collaborate with nursing, therapy, dietary, and social services to ensure assessments are complete and care plans are individualized.

RN license strongly preferred; LPN with significant MDS experience will be considered. This full-time role offers a comprehensive benefits package and a mission-driven work environment.

Qualifications

• MDS coordination should be demonstrated or strong relevant experience.

Responsibilities

• Lead the RAI process and drive accurate PDPM reimbursement.
• Collaborate with nursing, therapy, dietary, and social services to ensure assessments are complete.
• Develop individualized care plans and monitor CMS ARD windows.
• Educate staff on MDS documentation and coding practices.
• Support survey preparation and respond to MDS-related findings.
