Job summary

Fordland Clinic, Inc. invites an experienced healthcare leader to serve as PCHH Director & Case Manager. You’ll help patients navigate barriers, coordinate services, and connect them with community resources to stay engaged in care.

This full-time role requires an active Missouri RN license, strong clinical software experience, and a team-based approach. You’ll supervise health home staff, monitor performance, deliver training, and support enrollment and discharge processes while promoting

Qualifications

• Current RN license recognized by the Missouri Board of Nursing.
• Strong experience with clinical application software.
• Knowledge of care in accordance with regulations and practices established by the license board.
• Consistent demonstration of a team approach to care.
• Requires extensive knowledge of the PCHH program.
• Computer experience in database management, word processing, and spreadsheets required.

• Champions practice transformation based on health home principles.

Our PCHH program is built around a simple idea: patients shouldn’t have to navigate healthcare alone.

Our PCHH team works alongside patients to identify barriers, coordinate services, connect them with community resources, and provide the support they need to stay engaged in their healthcare. As the PCHH Director & Case Manager, you’ll help patients navigate those barriers, connect them with resources, coordinate their care, and make sure they don’t have to face the healthcare system alone. You’ll build relationships, advocate for patients, and help turn a healthcare plan into real-world progress.

Work hours: Full-time, Salary position This position can choose between 8-hour shifts 5 days a week or 10-hour shifts 4 days a week. One day per week in our Kimberling City office (paid for additional mileage) Clinic is open 8:00 am to 6:00 pm Monday through Friday and 8:00 am to 4:00 pm on Saturdays.
