Job summary

Maxim Healthcare in Modesto, CA is seeking compassionate home health aides to assist patients per plan of care, including daily living tasks and vital signs monitoring.

Qualifications

• Meets all training requirements per state and federal laws.
• Ability to read, follow written instructions and document care.
• Possess and maintains current CPR and First Aid certification.

• Valid driver’s license and vehicle insurance.
• Compliance with Maxim competency program and registries.

Responsibilities

• Assist patients per plan of care with daily living activities.
• Take and record temperatures, pulse, respiration, and blood pressure when ordered.
• Support safe transfers and ambulation.
• Provide exercise according to the plan of care.
• Administer medication reminders and monitor patient condition.
• Observe and report changes to the nurse supervisor.

Responsibilities

• Responsible for assisting patients with, and per plan of care, the following;
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Medication reminders
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR and First Aid certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Home Care Aide Registry Per ID

• Must meet all federal, state, and local requirements
