Job summary

Maxim Healthcare Services is seeking a Home Health Aide (HHA) to provide personal care to meet the patient’s needs as outlined in the plan of care. You’ll assist with daily living activities and document patient conditions for the supervising nurse.

Responsibilities include baths, dressing, transfers, ROM exercises, reminders for medications, and ensuring home safety. The role requires following the care plan and strong communication with the patient, family, and office.

Qualifications

• Meets all training requirements per state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• CPR and First Aid certification current per state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.
• Home Care Aide Registry per ID.
• Live scan fingerprint.
• Valid Driver's License and vehicle insurance.
• Meets all federal, state, and local requirements.

Responsibilities

• Assist patients with baths, oral hygiene, dressing, toileting, feeding, and related tasks per plan of care.
• Take and record temperatures, pulse, respiration and blood pressure when ordered (with competency).
• Provide safe transfers and ambulation, and assist with exercises per the care plan.
• Perform ROM and simple procedures as ordered (with competency).
• Provide medication reminders and monitor patient condition.
• Handle patient laundry and maintain safety with equipment (rails, stools, etc.).
• Observe and teach home safety practices; recognize emergencies and follow procedures.
• Follow the plan of care, complete tasks, and communicate clearly with client, family, and office.
• Observe and document changes and report to the assigned nurse supervisor.

Responsibilities

• Responsible for assisting patients with, and per plan of care, the following;
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Medication reminders
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
