Open Sky Community Services Inc. is seeking a Positive Behavioral Supports Clinician to lead the ASD Community Treatment Team across virtual, community, office, and family-home settings.
The role emphasizes collaboration with clinicians, nurses, coaches, consultants, and peers to implement training and evidence-based PBS practices. You will perform functional behavioral assessments, contribute to individualized care and safety plans, and ensure timely documentation to meet funder expectations
Serve as the primary clinician for the interdisciplinary ASD Community Treatment Team. Collaborate with all team disciplines including clinicians, nurses, clinical treatment team coaches, consultants, and peer specialists. Provide training for the community treatment team to include autism spectrum disorder, evidenced based and promising practices to include PACT, Trauma Informed Care and Positive Behavioral Supports. Establish relationships with people served, staff, families, and other stakeholders to develop shared understanding of person’s goals and dreams. Complete all required clinical assessments within 30 days of each individual’s admission to ensure timely and focused treatment planning. Conduct functional behavioral assessments. Support the ASD Community Treatment Team, individuals served and families in the development of individual care plans, safety plans and cope ahead plans. Ensure documentation is current and meets the program and funder requirements for data collection and success tracking towards goals which may include personal stability, community housing, employment, education, and positive family relationships. Ensure maintenance of an electronic confidential file for each person served, adherent to the file checklist, including required assessments. Participate in after hours on-call rotation to promote community access and stabilization for individuals served. Incorporate Positive Behavioral Supports (Universal Supports) into all elements of practice. Oversee the development of PBS Universal and Targeted interventions. Participate in treatment planning, coordination and provision of outreach and consultation to DDS, family members, and collateral outside service providers. Provide services within virtual platforms, in the community, in the office and within the family home. Develop, coordinate, and deliver training in conjunction with team members to address the clinical learning needs of collaborating service partners, as needed. Develop, coordinate, and deliver skill and therapeutic groups including PEERS Friendship and Dating, and the DBT informed Skills System. Develop positive, therapeutic relationships while maintaining professional boundaries and confidentiality with people served and families, staff provider agencies and funders. Build person centered non-judgmental supportive relationships. Serve as a member of the Open Sky Peer Review Committee, PBS Leadership Team, DDS Community of Practice, and other committees as assigned. Coordinate and lead the ASD Universal Support Team. Establish and maintain positive relationships and two-way communication with community team members/collaterals including funders, psychiatrists, therapists, direct support staff, and other stakeholders. Attend, participate, or facilitate meetings, training, and supervision, as assigned. Perform other duties as assigned.
