Role emphasizes person-centered relationships, collaboration with clinicians, and flexible crisis support. Candidates should bring a bachelor’s in human services with ASD/mental health experience; a master's may substitute for experience.
Job Description: Team Coaches will assist individuals in acquiring coping skills for anxiety, help develop self- advocacy, job readiness skills, awareness of public safety and develop tools for planning and executing functioning skills. They work with individuals served to increase interest and fluency in social and community activities and help them development independent living skills including personal hygiene, home maintenance, activities of daily living and other functional skills as assessed.
Bachelor’s Degree in Human Services or related field with a minimum of five years’ experience working with Autism Spectrum Disorder and significant mental illness, required. Master’s Degree in Human Services or related field will be considered in lieu of years of experience. Driver’s license, acceptable driving history and reliable transportation, required. Previous experience in assessment and treatment planning, preferred. Must be able to relate effectively to other staff members, as well as to provide stability and flexibility in crisis situations. Must possess leadership skills, flexibility, time-management, and effective verbal and written communication skills. Candidates must demonstrate the ability to meet with individuals using multiple modalities including virtual, in person (at the person’s home or community setting).
Establish relationships with persons served, staff, families and other stakeholders to develop shared understanding of person’s goals and dreams. Build person centered non-judgmental supportive relationships with individuals served. Assist individuals to acquire coping skills for anxiety, strategies for self-monitoring and escalation as needed. Assist individuals to increase interest and fluency in social and community activities. Assist individuals in developing self- advocacy, job readiness skills, awareness of public safety and develop tools for planning and executive functioning skills. Refer to community resources such as outpatient counseling and psychiatry. Assist individuals to obtain housing and entitlements. Assist individuals to development of independent living skills including personal hygiene, home maintenance, activities of daily living and other functional skills as assessed. Assist individuals to develop self-awareness of personal strengths and challenges, social skills and to recognize social cues and respond in an expected way across settings. Provide support to individuals regarding medications. Maintain communication with community team members and collaterals including psychiatrists, therapists, day program staff, etc. Consult to program participants and collaterals (staff/family/supporters) in identified supports, interventions & strategies. Develop productive affiliations and partnerships with other community-based providers to enhance the agency’s service delivery network. Attend, facilitate, or participate in trainings and meetings as assigned. Serve as a member of Open Sky Committees and task forces as assigned. Maintain complete and up to date confidential files for each program participant. Effectively utilize and maintain electronic health record. Participate with the clinician in the development of individualized treatment plans, Safety and Cope Ahead Plans with assigned individuals/families as clinically indicated. Participate in treatment planning towards the development of person centered, effective, and achievable treatment goals and objectives. Develop and monitor the progress of SMART objectives in collaboration with each individual served. Develop and facilitate community engagement opportunities and/or co-lead groups as assigned. Participate in the completion of comprehensive assessments & Life Plans with individuals served. Interface with other members of the treatment team including family members, DDS, day supports, professional and allied health providers including the VNA. Support the overall safety of individuals by providing support and resources in the time of crisis including evenings and weekends per on-call rotation. Perform other duties as assigned.
Pay Range: 27.95 - 27.95
