Job summary

St.-Bernards-Healthcare is seeking a Discharge Coordinator/Patient Care Technician to facilitate patient throughput and discharge processes. The role collaborates with case managers, RNs, physicians and the patient’s support system to implement the agreed discharge plan.

The position involves assisting licensed nursing personnel and supporting a range of operational discharge-planning activities in a hospital setting, with emphasis on accuracy, communication, and patient-centered care.

Qualifications

• High school diploma or equivalent required.
• Nurse Aide certification or RN/LPN student skills checklist completion preferred.
• Completion of approved Vo-Tech program or SBMC PCT program considered.
• On-the-job training and orientation will cover hospital policies.

Responsibilities

• Facilitate patient throughput and discharge processes as part of the healthcare team.
• Collaborate with case managers, RN, physician and others to implement discharge plans.
• Support discharge-planning activities and assist licensed nursing personnel as needed.
