Job summary

Recruiting.com LLC is seeking a Discharge Coordinator/PCT to support discharge planning and patient throughput in a hospital setting.

You will collaborate with case managers, RNs, physicians, and the patient/family to implement discharge plans, while performing assigned patient care tasks and providing essential operational support.

Qualifications

• High school diploma or equivalent is required.
• Nurse Aide certification or completion of SBMC PCT program preferred.
• Candidates enrolled in approved nursing programs may qualify via related checklists.
• Clerical and healthcare support experience is advantageous.

Responsibilities

• Facilitate discharge processes and coordinate patient throughput with the healthcare team.
• Collaborate with RNs, case managers, and physicians to implement discharge plans.
• Assist licensed nursing personnel and support operational discharge activities.

    Education

    
    

    Skills and proficiency in oral and written grammar, communication, and basic mathematics as acquired through official completion of high school or its equivalent. Ability to perform support activities in patient care as required through completion of an approved Vo-Tech program or certified as a Nurse Aide through the Office of Long Term Care, graduate of the SBMC PCT program, or after completion of an RN/LPN Nursing Student Skills Checklist completed by their nursing instructor. Candidates must be currently enrolled in an approved program to utilize the RN/LPN Nursing Student Skills Checklist. PCT experience within the last year will be considered in lieu of formal training.

  JOB SUMMARY

  
  

  The Discharge Coordinator/PCT serves as the facilitator of the patient throughput/discharge process and is a valued member of the healthcare team. They collaborate with case managers and other members of the healthcare team to implement the discharge plan agreed upon by the RN, case manager, physician, patient/chose support system, and other involved members of the interdisciplinary team. Responsible for performing assigned patient care activities and assisting licensed nursing personnel. The role is designed to support a wide variety of operational support functions that support discharge-planning activities.
