The Patient Services Coordinator, LPN/LVN acts as a vital link between clinical staff and patients, along with their families and caregivers. Responsible for scheduling, organizing, and managing all aspects of a patient's healthcare maintenance and treatment. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

  1. Manages schedules for all patients, edits schedules for agents calling in sick, ensures timely reassignment of patients, and updates agent unavailability in the worker console.
  2. Initiates infection control forms as needed, and sends the completed "Employee Infection Report" to HRD for upload in the worker console.
  3. Serves as backup during lunch hours and busy times, including receiving calls from field staff and assisting with weekly case conferences. Refers clinical questions to the Branch Director as necessary.
  4. Maintains the client hospitalization log, including entering coordination notes and distributing the electronic log to all relevant staff.
  5. Completes requested schedules for skilled nurse/injection visits, ensuring proper scheduling of supervisory visits and other related tasks.
  6. Runs scheduling reports, prepares weekly agent schedules, and verifies visit notes in the scheduling console.
  7. Assists with internal patient transfers between branch offices.
  8. If clinical, receives lab reports, assesses them, and routes copies to relevant parties, initiating infection reports as necessary.
  9. If clinical, may perform patient visits and participate in on-call rotations.

Required Experience/Skills:

  • Licensed Professional Nurse or Licensed Vocational Nurse licensed in the practicing state.
  • At least 1 year of home health experience.
  • Preferred: Packet review/QI experience, coding certification.
  • Valid driver’s license and automobile insurance.
  • Excellent communication, organizational skills, and ability to manage multiple assignments.

Scheduled Weekly Hours: 40

Pay Range: $45,400 - $61,300 per year, depending on location and experience.
