Base pay range

$45,400.00/yr - $61,300.00/yr

The Patient Services Coordinator-LPN is responsible for scheduling visits, communicating with staff, patients, and physicians to ensure proper care coordination and continuity. The role also involves assisting with daily office and staff management.

  • Manage patient schedules, update for sick agents, and reassign patients as needed.
  • Initiate infection control forms and communicate with HRD.
  • Serve as backup during busy times, handle calls from field staff, and assist in weekly case conferences.
  • Maintain hospitalization logs, enter notes, and distribute logs to staff.
  • Complete scheduling tasks, including discharge, recert, and add-on visits, ensuring supervisory visits are scheduled.
  • Run scheduling reports and prepare weekly agent schedules for approval.
  • Verify visit notes and assist with patient transfers between branches.
  • If clinical, handle lab reports, assess results, and participate in patient visits or on-call rotations as needed.

Required Experience/Skills

  • Licensed Professional Nurse or Licensed Vocational Nurse in the practicing state.
  • At least 1 year of home health experience.
  • Preferred: packet review/QI experience, coding certification.
  • Valid driver’s license and auto insurance.
  • Strong communication, organizational, and multitasking skills.

Scheduled Weekly Hours 40

Pay Range $45,400 - $61,300 per year
