Job summary

Mount Sinai Health System is seeking a Patient Services Coordinator II to serve as a liaison for patient care activities in the practice and ensure quality patient care during visits.

The role educates patients on care plans, coordinates services with Primary Care Providers and outpatient testing facilities, and prioritizes work to meet patient, family, and caregiver needs; collaborates with staff to facilitate care progression and improve satisfaction.

Qualifications

• 4+ years of healthcare/customer service or relevant experience.
• AAS degree or combination of education/relevant experience. Bachelor’s degree preferred. Medical Terminology Certificate highly desirable.

Responsibilities

• Works cohesively with the Primary Care Provider to facilitate patients’ care plan and scheduling.
• Collaborates with clinical staff to resolve issues impacting patient care progression.
• Monitors dashboards and coordinates reminders, follow-ups, and authorizations; resolves billing errors when applicable.
• Acts as liaison with physicians, patients and staff to connect patients with internal/external resources.

Job description

The Patient Services Coordinator II plays a crucial role in the delivery of healthcare services. The incumbent serves as liaison for patient care activities in the practice and ensures quality patient care during patient visits. The Patient Services Coordinator helps educate patients on their care plans and coordinate services with Primary Care Providers, and out-patient services diagnostic testing facilities. The Patient Services Coordinator II prioritizes work in alignment with the needs of the patients, family, and caregivers

• Works cohesively with the Primary Care Provider to facilitate patients’ care plan, participating in daily huddles/monthly case discussions, facilitating appointments, coordinating information and continuing care requirements.
• Collaborates with other clinical staff, to resolve issues impacting patients’ care progression.
• Monitors and updates dashboard with reminders and alerts, to organize daily tasks such as: visit status reminders, follow-up appointments, authorizations for medical review, coordination of specialty appointments and resolving billing errors (when app
• Contact insurance carriers for patients’ pre-certification; consults with Department/Practice Manager with respect to patients’ needs or addressing escalated issues.
• Provides front-end coverage as needed; records telephone encounters for routing to appropriate Providers.
• Works as liaison with Physicians, patients and clinical staff to connect patients with relevant internal/external resources.
• Provides recommendation for process/service improvement to promote patients satisfaction and address issues.
• Provide thoughtful and informative responses to patient while guiding them through their healthcare options.
• Maintain understanding of medical terminology and practices used within the hospital through further education.
• Performs other duties as assigned.

Compensation Statement

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $17.00 - $35.12 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
