Job summary

Inova Fair Oaks Hospital is seeking a dedicated Patient Safety Manager to lead system-wide safety initiatives and promote a just culture. This full-time, on-site role operates Monday through Friday during regular business hours, with sign-on bonus and relocation assistance eligible.

You will collaborate with leaders across the health system to advance harm reduction, analyze safety data, and deliver monthly TeamSTEPPS trainings as a subject matter expert.

Qualifications

• Bachelor's degree in related field (or equivalent experience).
• Minimum 5 years of healthcare experience with demonstrated quality/safety improvements.
• CPPS or CPHQ certification within 12 months of start.

Responsibilities

• Develops system-wide strategies to advance a safe, reliable culture and zero-harm to patients and staff.
• Facilitates daily safety reports and risk mitigation with executive leadership across the health system.
• Promotes psychological safety and accountable teamwork across departments.
• Leads improvements for harm reduction and clinical excellence using RCA², ACAs and FMEAs; conducts debriefs with improvement teams.
• Serves as SME and master trainer for monthly TeamSTEPPS team training.
• Coordinates peer support to assist staff after adverse events and maintain well-being.
• Analyzes safety data to drive improvements in reliable processes for harm prevention.
• Recommends enhancements to meet NPSG standards and regulatory readiness.

Patient Safety Manager Job Responsibilities

• Contributes to the development and execution of system wide strategies to support continuous advancement of a highly reliable safety culture for zero harm to patients and team members to support the organization’s mission, vision, and values.
• Facilitates daily situational awareness of safety reports across the entire health system for the executive team, working closely with all levels of the organization regarding event management, immediate risk mitigation, and robust long term sustainable improvements.
• Promotes and role models the way for a team environment that is committed to psychological safety as well as just and fair shared accountability.
• Leads process improvements for harm reduction and clinical excellence, using various safety evaluation methods such as RCA², ACAs, FMEAs; Debriefs process improvement teams.
• Serves as a subject matter expert and master trainer for monthly team training, using the TeamSTEPPS model for creating high performing teams.
• Coordinates peer support across the organization to assure team member support following adverse events that impact caregiver well‑being, resilience, and joy at work.
• Effectively analyzes and leverages safety event data to guide improvements in reliable processes for harm prevention.
• Recommends improvements for compliance with NPSGs to assure ongoing regulatory readiness.
• May perform additional duties as assigned

Minimum Qualifications

• Education: Bachelor's degree in related field (or relevant experience in lieu of degree equivalent to 4 years).
• Experience: 5 years of healthcare experience with demonstrated experience in quality/safety improvements.
• Certification: Certified Professional in Patient Safety within 12 months or Certified Professional in Healthcare Quality within 12 months

Preferred Qualifications

• Education: BSN
• Experience: Clinical inpatient experience;human factors engineering experience is a plus; ED experience is a plus
• Certification: CPPS upon start is a plus
• Other: strong interest in perf improvement
