Job summary

Saint Vincent Hospital is seeking a Patient Safety Officer to lead the facility-wide patient safety program. This role directs teams to reduce medical errors and improve patient outcomes, teaches system-based causes of errors, and communicates evidence-based safety strategies to management and staff.

The candidate will coordinate safety assessments, guide the safety function/committee, and ensure compliance with policies; strong leadership and communication are essential.

Qualifications

  • Bachelor’s degree in inpatient care field with J.D. (required).
  • 5 years direct patient care and 3 years in Quality/Risk/Patient Safety (required).
  • CPHRM or CPPS certification or eligible to take exam within two years (required).
  • Master’s in Nursing, Health Services, or Patient Safety preferred (optional).
  • ASHRM/NPSF/AHA/IHI curriculum completion preferred (optional).

Responsibilities

  • Investigates patient safety issues; leads Root Cause Analysis, Focused Reviews, and Intense Analysis.
  • Notifies Risk Management of risks; revises policies for patient safety.
  • Oversees annual scope of the Patient Safety Program; updates safety processes.
  • Ensures audits and monitoring per action plans.
  • Performs related duties as required.
