Job summary

Mass General Brigham invites applications for a role assisting patients with transportation to/from the hospital and locating affordable accommodations and social supports. You will work with the unit-based case manager to facilitate discharge planning to psychiatric or medical settings, and reference a broad range of community resources.

Qualifications

  • Education required: High School Diploma or Equivalent; Associate's Degree preferred.
  • Experience: 2–3 years preferred.
  • Familiarity with medical terminology and community resources.
  • Strong interpersonal, collaboration and advocacy skills.
  • Excellent written and verbal communication abilities.

Responsibilities

  • Research information on community social resources.
  • Maintain files of brochures, information packets and applications for community resources.
  • Collaborate with social work staff and community agencies to obtain information.
  • Assist in planning/implementing in-service presentations about community resources.
  • Assess patient/family understanding of available resources.
  • Educate patients/families about resources and how to access them.
