CommonSpirit Health is seeking a Patient Logistics Coordinator to facilitate patient placement across our facilities, working under the Patient Logistics leadership to ensure appropriate transfers and care coordination. You will use strong interpersonal and communication skills to coordinate patient movement, document activities clearly, and support transfer of care plans with care teams.

• Strong interpersonal and communication skills required.

• Experience in acute care or health care settings preferred.

Responsabilidades

• Works independently to facilitate placement of patients as directed.
• Facilitates transfer of patients to appropriate facility and unit.
• Identifies problems and alerts leadership to prevent barriers to timely placement/transfer.
• Communicates with referring and receiving facility staff to ensure timely placement.
• Documents activities clearly to promote effective communication with care team and providers.
• Uses effective communication with colleagues to facilitate timely patient placement.
