Job summary

Temple-Health is seeking a Patient Outreach Specialist to support Chestnut Hill Family Medical and Manayunk sites. The role handles patient communication, coordinates care, and serves as a liaison between families and providers.

The job requires at least a high school diploma; a Bachelor's in Marketing, Communications, or Healthcare is preferred, with 2 years in customer service or healthcare ideal. Full-time, day shift, onsite.

Qualifications

• High School Diploma or equivalent required.
• Bachelor's degree in Marketing, Communications or Healthcare preferred or combination of education/experience considered in lieu of degree.

Responsibilities

• Supports the practice sites by assisting with quality and operational tasks.
• Serves as point of contact between patients/families/caregivers and practice staff and providers.
• Communicates via oral and/or written correspondence to patients and physicians daily.
• Provides appropriate and clinically relevant information and facilitates requests within designated timeframes per protocol.
• Maintains knowledge of several IT systems and ensures database compliance and integrity.
