Temple Health is seeking a Patient Outreach Specialist to support Chestnut Hill Family Medicine and Manayunk sites. This 40-hour-per-week role serves as the primary contact between patients, families, caregivers and practice staff, handling inquiries and coordinating care in a timely manner.

The ideal candidate communicates clearly in writing and speech, maintains knowledge of multiple IT systems, and ensures data accuracy while facilitating requests in line with clinical urgency and protocol.

Qualifications

• Supports the practice sites by assisting with quality and operational tasks.
• Serves as a point of contact between patients/families/caregivers and practice staff and providers.
• Maintains knowledge of several IT systems and assures database compliance and integrity.

Responsabilités

• Acts as primary contact between patients, families and staff.
• Communicates via oral and/or written correspondence to patients and physicians daily.
• Provides clinically relevant information and facilitates requests within designated timeframes per protocol.
