Job Purpose

As an RN or LPN, the Patient Navigator serves our patients and families with care and respect while committing to advocate and better the lives of those we serve. Under limited direction, this position functions as part of the multidisciplinary team to coordinate patient care throughout the continuum of care and serves as a clinical resource for care management. The Patient Navigator is responsible for coordinating and facilitating patient needs including, but not limited to, internal and external patient transports, scheduling, and internal specialist rounds. The role requires flexibility and responsiveness to support the team while maintaining a strong focus on patients and families.

Work Schedule

Full-time, Monday - Friday, usually 8:00 a.m. - 4:30 p.m., with flexibility for early or late external patient transports (with notice).

Essential Duties & Responsibilities
  1. Coordinate healthcare needs: Connect patients and families to resources, support systems, and act as a patient advocate. (30%)
  2. Patient transfers: Perform safe and efficient transferring and escorting of patients. May require lifting, pushing, or pulling. (30%)
  3. Care coordination: Support primary and specialty care co-management through communication and follow-up. (30%)
  4. Communication and support: Serve as a contact point and informational resource for families, care teams, and specialists. (5%)
  5. Other duties as assigned. (5%)
Qualifications
  • Completion of diploma Nursing Program (LPN or RN)
  • Minimum of 2 years of experience as RN or LPN; pediatric experience preferred
  • Current LPN or RN license in Oklahoma
  • CPR certification within 1 year of employment
  • Valid driver’s license
  • Current BLS and PALS certifications (or ability to obtain within 1 year)
