Job summary

Akron Children's is seeking a PRN Patient Navigator to support our Bereavement Follow Up program and, as needed, Palliative Care initiatives. The role focuses on helping patients anticipate and overcome barriers within the health care system.

Responsibilities include coordinating care plans, communicating with families and care teams, documenting interactions, and working with physicians, nursing staff, and social work to address progress and concerns. Flexible hours, on-site and remote options.

Qualifications

  • High school diploma or equivalent required.
  • 0–2 years of relevant experience preferred.
  • Experience in a medical office environment is required.

  • Seeks assistance for appropriate social services and communicating with their treatment team, family, and significant others.

  • Helps to modify and enhance the patient navigation process by incorporating the barriers identified in the community needs assessment.
  • Other duties as required.

Technical Expertise
  • Experience in a medical office environment is required.
  • Experience working with all levels within an organization is required.
  • Experience in healthcare is preferred.
  • Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.
Education and Experience
  • Education: High school diploma or equivalent is required.
  • Certification: None
  • Years of relevant experience: 0 to 2 years is preferred.
  • Years of experience supervising: None.
