About the Role

As a Palliative Care Nurse Navigator, you will support patients and families throughout their healthcare journey. Your responsibilities include coordinating care between inpatient and outpatient settings, participating in hospital rounds, conducting patient follow‑up, facilitating transitions to hospice services when appropriate, providing education to patients and families, and serving as a resource for providers and clinical staff.

Essential Functions
  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals and support for follow‑up care for patients who are uninsured or underinsured.
