Job summary

Mount Sinai Medical Center is seeking an Oncology Navigator (Generalist) with certification to be obtained within 1 year of hire. The role focuses on guiding patients through cancer care, coordinating services, and ensuring timely access to treatments.

Responsibilities include barrier identification, documentation, tumor board participation, patient education, and collaboration with physicians and care teams to streamline care pathways across the cancer center.

Qualifications

• License/Registration/Certification: None
• Education: Bachelor's degree in related discipline preferred
• Experience: 3-5 years in patient/healthcare relations or human services preferred

Responsibilities

• Identifies barriers to care and refers patient/family to nutrition services, social services, integrative medicine, or psycho-oncology appropriately and promptly.
• Maintains documentation of all patient encounters and complete reporting requirements.
• Flexible and adaptable in response to changing patient and healthcare providers' needs.
• Attends and represents the organization at professional conferences, in-service trainings, and meetings with supervisor approval.
• Regularly participates in disease-specific and general tumor boards and monthly interdisciplinary meetings.
• Supports the patient/family on their cancer journey from pretreatment education, active treatment, survivorship and end-of-life care.
• Collects, analyzes and reports clinical navigator outcome data, metrics, and productivity benchmarks with administration and cancer registry staff.
• Provides patient education to assist with informed decision making, timeliness of care, and multidisciplinary care processes.
• Works with physicians/APPs and other healthcare team members for pathology and molecular lab testing requests and results.
• Facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and continuity of care.
