Job summary

Mount Sinai Medical Center is seeking an Oncology Navigator Generalist to support cancer patients through diagnosis, treatment, and survivorship. The role emphasizes removing barriers to care, coordinating with nutrition, social services, and psycho-oncology, and documenting all patient encounters.

The Navigator will work with physicians, APPs, and the cancer registry team, collecting outcome data and assisting with education to empower informed decisions and timely care.

Qualifications

• Bachelor’s degree preferred in related discipline.
• 3–5 years experience in patient/healthcare relations or human services field preferred.
• Oncology Navigator certification to be obtained after 1 year from hire.

Responsibilities

• Identify barriers to care and refer patients to nutrition, social services, integrative medicine, or psycho-oncology in a timely manner.
• Maintain documentation of all patient encounters and reporting per organization standards.
• Attend professional conferences, in-service trainings, and meetings as requested by supervisor.
• Participate in tumor boards and interdisciplinary meetings to support coordinated care.
• Support patient/family throughout cancer journey from pretreatment to survivorship and end-of-life care.
• Collect, analyze, and report navigator outcome data with administration, IT, and cancer registry staff.
• Provide patient education to aid informed decision making and improve timeliness of care.
• Collaborate with physicians/APPs for pathology and molecular testing requests and results.
• Facilitate scheduling of appointments, testing, and procedures to promote continuity of care.
