Evolving Solution Services in Hartford, CT is seeking a Patient Navigator LPN to guide patients through the healthcare system and connect them with resources for timely care.

You will work with physicians, nurses, and care teams to coordinate services, maintain accurate records, and educate patients and families about treatment options.

This full-time, non-exempt role requires an active LPN license, at least two years in healthcare, and strong communication, organizational, and teamwork skills.

Salary Range: $30.00 To $31.00 Hourly

About the Role:

As a Patient Navigator LPN, you will play a crucial role in ensuring that patients receive the best possible care. You will be responsible for guiding patients through the healthcare system, providing them with the support and resources they need to navigate complex medical procedures and treatments. Your primary goal will be to ensure that patients receive timely and effective care, and that their needs are met throughout the entire healthcare process. You will work closely with physicians, nurses, and other healthcare professionals to coordinate care and ensure that patients receive the highest quality of care possible.

Minimum Qualifications:
  • Active LPN license in the state of employment.
  • Minimum of 2 years of experience in a healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent organizational and time management skills.
Preferred Qualifications:
  • Experience working as a patient navigator or care coordinator.
  • Experience working with electronic medical records (EMRs).
  • Bilingual in English and Spanish.
  • Experience working with underserved populations.
