Essen Health Care is an integrated healthcare delivery organization that provides high-quality, compassionate, and accessible medical care to many under-served residents of New York State. Guided by a population health model of care delivery, Essen’s patient-centric approach focuses on meeting patient needs across all care settings.

Founded in 1999, Essen clinicians provide care in all five boroughs of New York City, with 150 primary and specialty care physicians and nearly 100 advanced clinicians.

The Patient Navigator will provide care coordination services to members with chronic health conditions, acting as an intermediary between providers and members. Responsibilities include education, evaluation, service coordination, community resource research, and more.

1. Conduct outreach activities, especially to patients lost to care.
2. Perform community outreach visits to patients.
3. Provide expedited visits for urgent situations such as hospitalization.
4. Assist in a busy Medical Office environment.
5. Call, schedule, and confirm patient appointments.
6. Enroll patients in Essen Health Care’s programs.
7. Help reduce emergency room visits and hospitalizations, improve service coordination, and navigate social services.
8. Conduct new patient assessments according to the Scope of Service.
9. Inform patients about Essen services and provide health education materials.
10. Keep patients informed about the progress of scheduled appointments.
11. Meet with patients in their homes, community sites, doctor’s offices, and the Health Home office.
12. Work in the field as required.

Minimum requirements include a High School Diploma or Associate Degree in Health Care, at least 2 years of experience in healthcare, community health, social work, or related fields, excellent customer service and communication skills, proficiency in Word and Excel, and the ability to multitask in a fast-paced environment.

The position offers an hourly rate of $19-$20.
