Job summary

Apicha Community Health Center in Manhattan, NY seeks a full-time Patient Navigator to provide field-based case management for patients enrolled in HIV care coordination programs. You will be the main contact and help overcome barriers to care, coordinating transportation, appointments, and resources with a care team.

The ideal candidate has an Associate’s degree in human services or social work and at least 12 months of navigation experience, with strong English communication and cultural

Qualifications

  • Associate’s degree in human services or social work and at least 12 months experience in patient navigation or similar field.
  • May consider certification as a peer worker by the NYS AIDS Institute Peer Certification, or CUCS case management certification, or at least 90 hours of documented training equivalent to those required by the NYS Peer Worker.
  • Strong socio-cultural identification with the target population or experience in working with target population required.
  • Fluency in one or more Asian language preferred.
  • Excellent written and verbal communication skills in English required.
  • Ability to communicate well with medical providers and support staff.
  • Ability to work well with diverse population.
  • Ability to handle multiple tasks and stressful environment.
  • Ability to work well within the organizational structure.
  • Ability to work flexible hours including some evenings and weekends.
  • Ability to use common office software (Word, Excel, Power Point). Familiarity with eSHARE preferred.

Responsibilities

  • Provide direct services to person who living with HIV/AIDS under supervision of Care Coordinator.
  • Carries out tasks that are needed to execute the medical and support service plans, including accompanying patients to appointments, coaching, delivering health education encounters, reassessment, and coordinating logistics for plan adherence reminders, transportation and childcare arrangement.
  • Responsible for field-based tasks: observes and records patient self-administration of ARVs, assess for and reports ARV related side-effects, and deliver in home, CBO, work or other convenient locations for PLWHA.
  • Maintain client-related records and other required documentation according to protocols and standards.
  • Working as part of the unit on quality assurance and CQI tasks.
  • Represent the agency at various venues to promote agency services.
  • Other activities as assigned.

Position Summary:

We're looking for a committed, professional full-time Patient Navigator for our Manhattan location. The ideal candidate is someone who is personable, works well with a diverse population, able to communicate effectively well with medical providers and support staff and has good attention to detail. The Patient Navigator provides field-based case management services to patients enrolled in the HIV care coordination program. Patient Navigator is the main point of contact for the patient. The Patient Navigator builds strong relationships with HIV positive patient in order to help them connected and to stay engage in prevention and treatment services. Patient Navigator is committed to removing the patient’s barrier to care by identifying critical resources and helping them navigate through health care services and system and to promote patients’ health. Patient Navigator works closely with the care team which may include doctors, nurses, and other staff in the care team to ensure the patient’s optimal health care outcomes.

Job Responsibilities:

  • Working as a part of the unit at the quality assurance and continue quality improvement (CQI) tasks.
  • Represent the agency at various venues to promote agency services.
  • Other activities as assigned.
