Job summary

Housing Works in New York is seeking a Care Coordination program professional to address HIV healthcare disparities through client-centered, holistic care management.

Job description

  • Pay or shift range: $20 USD to $23 USD The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

Compensation Range: $20/hr - $23/hr commensurate with experience

Benefits: We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

Overview:

The Care Coordination program seeks to address HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approach to meet the needs of people with HIV with team-based care management. The program makes use of a navigation approach to identify, advocate for, and coordinate the complex healthcare and social service systems necessary to ensure improved outcomes among people with HIV. Patient navigation guides clients in knowing where, when, and how to access healthcare and related services by improving access to resources and providing individualized support to address barriers to care.

Responsibilities:

Primary Responsibilities Include:

  • Provide client – centered, culturally and linguistically competent services
  • Support the delivery of care and services to clients by facilitating adherence to scheduled appointments and care regimens
  • Refer clients to appropriate care and provide linkage support
  • Provide accompaniment to health care and social service appointments
  • Conduct community-based home visits in a variety of settings, including private residences, shelters, and other congregate living facilities.
  • Collaborate with key stakeholders at each site, including leadership clinic staff, program staff to ensure effective communication and service delivery
  • Deliver health education curriculum to increase self-efficacy and health management skills
  • Provide tailored medication adherence support

Other functions that may be required with this position include:

  • Participation in Housing Works advocacy events

Requirements:

  • Highschool diploma and 2 years of relevant experience related to healthcare or social services
  • Ability to interact with inter/disciplinary team including medicine, social work, psychiatry, nursing, administration, and other paraprofessionals
  • Ability to work with and understand issues of people with HIV/AIDS, mental illness and substance use populations
  • Knowledge of and experience with mediation and conflict resolution
  • Good verbal and written communication skills
  • Embraces a harm reduction approach to working with underserved populations
  • Experience with eSHARE and/or eClinical Works is preferred
  • Bilingual English/Spanish preferred, but not required
