Job summary

Strive Health is hiring a Patient Navigator in Michigan to support the care team by engaging patients over the phone and in person, enrolling them in our kidney-focused care program, and guiding them through their health journey.

You will travel within the designated territory, meet with patients at clinics or homes, and build strong relationships while educating them about our care model and enrollment requirements.

Qualifications

• High School Diploma required.
• 4+ years in customer/patient-facing role.
• Experience promoting and selling services to end users.
• Willingness to travel up to 80% and travel within 100 miles of territory.
• Driver’s license and working vehicle required.
• Internet speeds min 3.8Mbps/3.0Mbps with latency under 60 ms.
• Ability to travel and be on-site as needed.

Responsibilities

• Proactively outreach to current and prospective patients via face-to-face activities and outbound calls to educate and enroll in care.
• Build long-term patient relationships and educate on enrollment requirements.
• Meet or exceed daily outreach, connections, and enrollments targets.
• Present Strive as an educational and empathetic resource for our care model.
• Maintain up-to-date CRM data (Salesforce).
• Collaborate with nephrologists and staff to build credibility and access to patients.
• Manage scheduling to achieve quarterly targets.

What You’ll Do

The Patient Navigator is responsible for supporting Strive’s care team to keep patients engaged with our complete care program specifically designed for people with kidney conditions. While most of your patient interactions will come by phone, there will be opportunities to meet patients face to face at their office visits or in their home. Success is measured by scheduling patients for program appointments which will occur through building strong relationships, being an educational resource, and helping navigate questions.

Essential Functions

• Proactively outreach to both current and prospective patients primarily through face-to-face activities and outbound phone calling to educate them on the care services available to them and enroll them within an assigned geographic area.
• Build trusting, long-term relationships with patients and appropriately educate on the requirements to enroll with Strive.
• Meet or exceed daily outreach expectations towards outbound calls, connections, and enrollments.
• Present yourself as an educational and empathetic resource to potential patients about our care model that is intentionally built for them.
• Build key internal relationships to elevate patient support needs.
• Face-to-face patient outreach which can include; at home door knocking, at their clinic visits with primary care or specialists, at events or other in-person engagement opportunities as needed.
• Meet with partner nephrologists and practice staff to build credibility of Strive, earn the right to meet patients at their locations.
• Maintain up-to-date client information in customer relationship management (CRM) tool (i.e., Salesforce).
• Maintain long-term relationships with patients (even after enrollment) to positively support patient retention and satisfaction.
• Support the development of established external partner relationships.
• Meetin personwith internaland/orexternal stakeholders to facilitateteam and businesspriorities/opportunities.
• A positive and proactive attitude towards building a business.
• Own your schedule to effectively manage day-to-day responsibilities required to achieve a quarterly target.
• Other duties as assigned.

Minimum Qualifications

• High School Diploma.
• 4+ years’ experience working with customers or patients in a professional setting.
• Experience with promoting and selling services to end users.
• Experience in customer service, sales or a similar customer facing role.
• Business travel required up to 80%, some overnights.
• Driver’s license and working vehicle required for home visits and travel within 100 miles of designated territory.
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms.
• Ability to travel and be on-site to meet business needs.

Preferred Qualifications

• Bachelor’s Degree.
• Experience with promoting and selling primary care or other healthcare related services to high needs populationspreferred.
• Experience in healthcare, specifically educating patients on care models or insurance plans preferred.
• Community health experience.
• Empathy for the burden of kidney disease.

Annual Salary Range: $51,000 - $60,000

Final compensation will be determined based on location, experience, and qualifications.
