Job summary

Silver Cross Hospital in Lemont, IL is seeking a Patient Navigator to coordinate patient care delivery and testing across assigned offices. You will identify needs, prioritize responses, and support efficient clinical staffing and care flow.

• Must maintain current CPR and BCLS certifications.
• Phlebotomy license or experience preferred.

Responsibilities

• Deliver patient care and manage patient flow across assigned offices.
• Coordinate patient lab draws and diagnostic testing (EKG, spirometry, etc.).
• Identify provider, office and patient needs and respond promptly.
• Promote throughput and deploy clinical staff as needed for efficiency.
• Maintain positive relationships with employees, providers, and hospital departments.
• Assist Medical Assistants as necessary.

Summary:
The Patient Navigator is responsible for delivery patient care delivery and management. Coordinates patient lab draws and other diagnostic testing such as EKG, spirometry, etc. for the assigned offices and providers. Identifies current provider, office and patient needs, prioritizes and responds accordingly. Focuses on throughout and efficiency deploying clinical staff and themselves needed. Ability to effectively evaluate and manage clinical caregivers. Maintains positive and effective relationships with employees, providers, hospital departments, and the public. Assist Medical Assistants as necessary.

Requirements:

Minimum 5 years previous primary care Medical Assistant experience required. Must maintain current CPR and BCL certifications. Phlebotomy license or experience preferred.
