Job summary

ChenMed, a quickly expanding primary care provider, seeks a Manager, Patient Navigation in Miami to lead day-to-day operations of a centralized navigation function. You will manage Tier 1 and Tier 2 staff, ensure CMS delegation adherence, and partner with a borrowed Medical Director for Tier 3 sign-offs.

Your role includes monitoring performance metrics, coaching staff, and coordinating with clinical and enrollment teams to ensure seamless patient care across all channels.

Qualifications

• BA/BS in Healthcare Administration, Business, or related field.
• 3–5 years in healthcare operations, call center management, or patient services; supervisory experience.
• Experience in Medicare Advantage or managed care environments preferred.

Responsibilities

• Lead day-to-day operations of the Patient Navigation function with Tier 1 and Tier 2 staff.
• Oversee intake and triage across three in-scope workstreams to ensure closed-loop documentation.
• Perform call monitoring and audits to ensure CMS and delegation standards compliance.
• Coordinate with Medical Director for Tier 3 clinical sign-off; manage review cadence and volume.
• Track and report team performance to senior leadership.
• Escalate patterns affecting patient risk scores or costs promptly.
• Coach and develop staff; lead training on processes and compliance.
• Collaborate with clinical and enrollment teams on hand-offs beyond Phase 1 scope.
