Job summary

Trinity Health MercyOne is seeking a Patient Logistics Coordinator to serve as the first point of contact for transfer, admission, and consultation requests. You will collect and verify demographic, referral, and operational information from sending facilities and EMS agencies, while managing multi-line phone activity and routing information to the appropriate clinicians.

Responsibilities include documenting call details in EHR and transfer management systems, coordinating non-clinical transport

Qualifications

• Health care experience required.
• High school diploma or GED required.
• Associate’s Degree or equal experience preferred.

Responsibilities

• Serve as the first point of contact for incoming transfer, admission, and consultation requests
• Collect and verify demographic, referral, and operational information from sending facilities, EMS agencies, and providers
• Manage multi-line phone activity, prioritize calls, and minimize delays
• Document call details and required information in electronic health records and transfer management systems
• Route calls and essential information to Patient Logistics Clinicians, RNs, Charge RNs, or operational team members
• Support EMTALA-sensitive processes through complete and timely intake documentation and escalation of red flags
• Coordinate non-clinical transport logistics, including contacting EMS agencies, confirming availability, monitoring ETA updates, and communicating status changes
