Job summary

HCA Healthcare's Patient Logistics Specialist position in Richmond, VA offers a critical role in coordinating patient transfers, bed availability, and disposition across Emergency Departments and inpatient settings. You will respond to calls, emails, and faxes, gather information, and support rapid patient flow with clinical insight and urgency.

The role requires 2+ years in healthcare, experience with transfer centers or bed management, and will be scheduled for full-time nights with weekend

Qualifications

• 2+ years of healthcare experience highly preferred
• Behavioral Health experience highly preferred
• Call Center - Transfer Center - Bed Management experience helpful

Responsibilities

• Responds to phone calls with urgency and good customer service
• Gathers initial information to open new case/call and prepares clinical packets
• Helps locate resources and coordinates information to support patient disposition/admissions
• Maintains active bed availability inventory within the community for non-HCA bed placements
• Responsible for accepting, tracking and facilitating hospital transfers and transports telephonically and via computer applications
• Provides updates to customers on status of patient cases (assessment status and bed placement)
• Assists to schedule and prioritize discharge transports and supports rapid response to requests
• Demonstrates ability to assess patient acuity and facilitate patient flow through the continuum of care

The Patient Logistics Specialist is responsible for the intake of new case referrals including status updates to/from Emergency Departments, Inpatient Hospital Settings, or Community Settings. The Patient Logistics Specialist responds to phone calls, faxes, and e-mail requests, or other forms of intake communication, for clinical information, patient admissions or transfers, transport requests and other non-clinical information.

Primary Duties and Responsibilities

• Responds to phone calls with a sense of urgency and, demonstrating good customer service skills

• Gathers initial information required to open new case/call(s) and prepares clinical information/packet which is to provide to the clinicians for assessment and next steps in advancing patients care.

• Helps to locate available resources and coordinates information to support patient disposition and or admissions to appropriate unit/facility.

• Maintains active inventory of bed availability within the community for non-HCA bed placements and admissions.

• Responsible for accepting, tracking and facilitating hospital transfers and transports telephonically and via computer applications.

• Provides updates to customers on status of patient case(s), i.e. assessment status and bed placement requests

• Assists to schedule and prioritize discharge transports and supports the Patient Logistics Center by assuring rapid response to requests for services

• Demonstrate the ability to process patient acuity utilizing critical thinking skills, clinical decision making, and acquired knowledge to assist in assessing situations and facilitate patient flow through the continuum of care.

Experience

• 2+ years of healthcare experience highly preferred

• Behavioral Health experience highly preferred

• Call Center - Transfer Center - Bed Management experience helpful

Scheduling Requirements

• This position is Full-Time Nights and includes a weekend rotation: 12 hour shift requirement unless otherwise noted. This role will require you to work some holidays.

Location Requirements

• You will perform the duties and responsibilities of this position onsite at an HCA Patient Logistics Center (PLC).

• Qualified candidates must live within 60 miles of an HCA PLC in the market area associated with the position | Jacksonville, FL
