Qualifications

• Graduate of an accredited nursing school.
• Associate degree required; BSN preferred.
• Current Connecticut RN license.
• BLS certification by the American Heart Association.
• Excellent assessment and critical thinking skills.

Responsibilities

• Implements and monitors patient care plans; documents patient status.
• Serves as primary coordinator of multidisciplinary patient care.
• Follows physician and nursing orders accurately.
• Assesses and coordinates discharge planning with the healthcare team.
• Identifies patient needs across physical, psychosocial, cultural and age-related domains.
• Delegates care tasks to nursing staff appropriately.
• Participates in daily interdisciplinary patient rounds and provides input.
• Adheres to ANA standards and codes of ethics and professional guidelines.
• Develops and revises individualized care plans.
• Engages in professional growth activities and continuous development.
