Height Street Skilled Center in Bakersfield, CA is seeking a dedicated Physical Therapist to join our rehabilitation team in a skilled nursing setting. You will evaluate, treat, and coordinate care to help residents improve mobility and independence.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California PT license.
• CPR certification.
• Experience in a skilled nursing or long-term care setting preferred, but not required.

Responsibilities

• Evaluate and assess patients to develop individualized treatment plans
• Provide physical therapy services to patients recovering from injury, illness, or surgery
• Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients, families, and caregivers on therapy techniques and home exercises
• Maintain accurate and timely documentation in accordance with state, federal, and facility guidelines
• Ensure compliance with all regulatory and facility standards
• Contribute to team meetings and care planning for residents
