Job summary

Jefferson Health in the Philadelphia region seeks a Patient and Family Centered Care Coach to advance compassionate care through staff coaching, data-driven insights and service excellence. This on-site role supports Albert Einstein Medical Center by embedding patient-centered practices into daily operations.

You will lead workshops, analyze patient-experience data, and collaborate with leaders to implement actionable plans that enhance care and engagement across clinical and non-clinical teams.

Qualifications

• Bachelor’s degree required in nursing, healthcare administration, organizational development, communications, psychology or related field.
• 3 years healthcare system/organizational improvement, patient experience, and training experience preferred.

Responsibilities

• Educates colleagues on patient-experience projects and creates curriculum based on leadership action plans.
• Facilitate workshops and training sessions on patient-centered care, service recovery, communication and empathy.
• Analyze patient experience data (e.g., HCAHPS, Press Ganey) to identify trends and opportunities.
• Collaborate with department leaders to execute targeted action plans for improvement.
• Embed patient-centered principles into daily operations and advocate for patient/family perspectives.
• Manage accountability and sustainability reporting tools and systems.
• Model behaviors through rounding and feedback; assist with service recovery and escalation.
• Participate in committees/projects related to culture, quality, safety and experience improvement.
