Job summary
Duo Health in San Juan, Puerto Rico is seeking a Patient Experience Specialist to engage patients and support care team operations by serving as the first point of contact for patients attributed to Duo Health. This role is ideal for a proactive, relationship-driven individual who excels at communication, building trust, and guiding patients through the initial stages of care engagement.

Qualifications
- Minimum 3 years in customer service, healthcare, or related field.
- Experience with outbound calls, patient engagement, or community outreach preferred.
- Bilingual in Spanish and English (written and verbal) required.
Responsibilities
- Conduct outbound calls to patients to introduce services and build awareness.
- Be the first point of contact, establishing trust with patients and caregivers.
- Schedule initial visits with Duo Health care team providers.
- Maintain follow-up with patients not reached or hesitant to engage.
- Document all patient interactions in the EMR and tracking systems.
- Collaborate with care teams to onboard patients and ensure smooth transition.
- Meet patient engagement goals and participate in community outreach events.
- Educate the community about Duo Health services and enroll eligible patients.
